InfoMC Technology Resources to Support Population Health Management

September 20th 2016

InfoMC Coordinated Care Solution Exceeds EHR Capacity for Care Coordination

Conshohocken PA (September 20, 2016) -- InfoMC, a leading provider of cloud-based healthcare management and care coordination software, enhances its identification and stratification model, InSpotlight, for Population Health Management. InfoMCs InSpotlight provides the tools and resources for identifying, stratifying, and analyzing at risk populations, and establishes the necessary care coordination and care management workflows to support effective care and quality outcomes for at-risk and chronically ill individuals.

The InfoMC InSpotlight tools accommodate large data sets of medical and behavioral health claims, pharmacy and lab data, service utilization, and other key indicators of population health.  A common challenge with health analytic tools are their inability to integrate physical and behavioral health indicators at a sufficiently granular level to guide care coordination and management.  InSpotlight integrates the full spectrum of physical, behavioral and social determinants, and targets select populations for improved health management and outcomes.  This is accomplished through the identification of key elements of the covered population; stratification of the population through a rules based approach to determine both high risk and those most likely to benefit from care coordination and management; and custom analytics to support the unique needs of different populations and provider systems.

Dr. Susan Norris, InfoMCs Chief Clinical Officer notes "We are excited to introduce InSpotlights enhanced identification and stratification model to our clients and the market. By utilizing physical, behavioral and social health as determinants, we are provided with the whole-person view of at-risk and complex individuals. The enhanced InSpotlight model then takes it a step further by auto-generating effective patient-centered care coordination workflows to support better health outcomes for these individuals and their unique needs."

The InSpotlight stratification model is based on 8 established key health indicators.  These indicators integrate both physical and behavior health factors, and can be weighted for different ratios based on the target population and health system preference.  A 9th health indicator supports client based rules for targeting select populations that can integrated into the overall InSpotlight analytics.  This can be useful for the management of specific populations and risk based contracts.

The Healthy Behavior Change E-Guide is rooted in years of medical and social research supporting the premise that whole-person health and the behavior change needed to achieve that goal must engage resources beyond the traditional healthcare system. As my board term comes to a close, I am particularly pleased by the projects recognition of the importance of health information technology as an essential component for individuals, healthcare providers, and the industry as a whole to improve health and save lives," says Mr. Farook.

The Healthy Behavior Change E-Guide has been recognized and praised throughout the healthcare industry by patients, providers, professional and trade associations, and government entities alike as a valuable resource for the empowerment of people, families and communities to take charge of their health.

InfoMC Brief: Closing Health Care Gaps Through Care Coordination

March 22nd 2016

System Redesign and New Technology Resources Drive Innovation and Change

March 22, 2016 [Conshohocken PA] -- InfoMC, a leading provider of cloud-based healthcare management and care coordination software, released today the InfoMC Brief, "Closing Health Care Gaps Through Care Coordination". The brief asserts that historical attempts to bridge the gaps between physical, behavioral and social determinants have in fact perpetuated silos between providers and achieved little to improve the patient experience across systems of care.

To close those gaps, InfoMC believes fundamental changes must occur in how health systems are designed and how priorities are set. And while electronic health records (EHRs) are effective tools in recording patient history, they often fall short in fully supporting the coordination of care across all physical, behavioral and social determinants of health.

The InfoMC Brief identifies four key aspects of health care that must change in order to close those gaps:

  • New approaches for understanding patients and diagnosing their health conditions
  • New technologies to achieve care coordination among all healthcare providers and social supports
  • Redefine payment and reimbursement models to reward quality of care/outcomes, not volume of service
  • Promote patient-focused empowerment to foster informed health decisions and health behavior change

InfoMC CEO JJ Farook says, "Its been over 15 years since IOM released its seminal review of the American healthcare system, "Crossing the Quality Chasm", and it remains a highly respected guidepost for the industry. While there have been great strides in moving towards a more patient-centered system, it is still very fragmented. The InfoMC Coordinated Care Solution uses state-of-the-art technology to eliminate disconnects and disparities between systems of care, provide true care coordination, and produce quality outcomes. Were not just bridging the gaps, were closing them."

The InfoMC Brief, "Closing Health Care Gaps Through Care Coordination", can be downloaded in its entirety on the InfoMC website at InfoMC Briefs.

InfoMC Publishes New Health Information Technology Tools and Resources

Blog & White Papers Address Top Concerns in Patient Care and Data Analytics

February 15, 2016 [Conshohocken PA] -- InfoMC, a leading provider of cloud-based healthcare management and care coordination software since 1996, has launched two new industry communication tools designed to identify and explore in-depth current and emerging challenges and opportunities in today's rapidly evolving healthcare environment.

The white paper, Why Electronic Health Records are Ill-Suited for Population Health, appeared in the February edition of the Healthcare Solutions Showcase from FierceHealthPayer (fiercehealthpayer.com), an online news service program focused on the fast-changing and heavily regulated area of healthcare reimbursement with more than 38,000 subscribers.

The white paper, authored by Susan Norris, Ph.D., Senior Vice President, Clinical Solutions for InfoMC and Allen S. Daniels, Ed.D., Clinical Director for InfoMC, takes a deep dive into the limitations of electronic health records (EHR) systems to track, access, and share health information documented outside of the clinical systems they support. The paper advocates for a care coordination approach to improve healthcare outcomes and reduce costs, and stipulates that effective care coordination is only achieved when technology addresses the existing gaps in patient care. In a side-by-side comparison of EHRs and Health Management Systems, the paper identifies the key functionality and operational efficiencies required to close the data sharing gaps across multiple provider systems, and establish workflows that direct and support the full spectrum of healthcare, social systems, and others who care for individuals.

InfoMC has also launched a health information technology blog, InfoMC Spotlight, for review and commentary on emerging trends, challenges and opportunities in the rapidly evolving healthcare marketplace. The first post, responds to a recent article in HealthLeaders Media identifying four priorities for healthcare payers in 2016. The post, "Effective Technology Solutions for Emerging Healthcare Priorities,"addresses each priority with a brief analysis of the underlying problem and recommends the type of technology infrastructure and capacity needed to successfully meet current and future demand.

InfoMC Spotlight posts and published white papers are also available for viewing or download at InfoMC News and Events.

July 2nd 2015

Rob Kagarise, Former Trinity Health Executive, Joins InfoMC Management Team

July 2, 2015 [Conshohocken PA] -- InfoMC, a leading provider of collaborative process improvement technology for healthcare organizations, is pleased to welcome Rob Kagarise to the InfoMC leadership team. Mr. Kagarise holds the position of Senior Vice President of Operations.

Mr. Kagarise's professional experience includes over 20 years of senior management roles in healthcare and professional services organizations. As Senior Vice President of Operations at InfoMC, he is directly responsible for the development and execution of the application roadmap, delivering exceptional value to customers and ensuring that effective operational practices and controls are in place.

InfoMC CEO, JJ Farook, says, "We are excited to have Rob on board. His knowledge and expertise will expand and improve upon the paths we have already forged in the areas of population health management, chronic care coordination, and data analytics. Every healthcare technology publication and industry authority I know has commented on the necessity and importance of these critical components of a successful healthcare system. With the recent Supreme Court upholding of the Affordable Care Act and its inherent supports and mandates for population health management programs and data collection, analysis, and sharing requirements - and now with Rob's strategic leadership - InfoMC will lead the way in bringing advanced technology solutions to market."

"InfoMC is blazing new trails in coordinated care management technology and helping organizations succeed and thrive in an increasingly competitive yet tightly integrated industry. Without the ability and willingness to participate in and contribute to data registries and information exchange, healthcare organizations, regardless of where they reside and operate within the system, can't meet the growing consumer demand for high-quality, collaborative care and patient engagement. I am delighted to be a part of such a forward-thinking company and look forward to contributing to many more technical advancements," says Rob Kagarise.

Before joining InfoMC in 2015, Mr. Kagarise held leadership roles at Trinity Health within the Population Health, IT Strategic Planning and PMO areas during a 10-year span. Under his leadership, Trinity Health successfully developed and deployed enterprise wide population health, interoperability and analytics solutions to meet the challenges of Healthcare Reform. A key focus of his work was supporting a number of commercial and Medicare ACOs through the deployment of core capabilities including patient\provider portals, quality measures, clinical alerts, point-of-care registries and a care coordination platform. Prior to joining Trinity Health, Mr. Kagarise spent a decade at Deloitte Consulting providing implementation and strategic planning services to organizations across a number of industries including healthcare.

PACE@Home Selects InfoMC Technology for Dual Eligible Program

September 11th 2012

Incedo Helps North Carolina Senior Day Services Manage Clinical and Business Processes

Media Contact:  Angela Madara, angelam@infomc.com, 484.567.0603

September 11, 2012 – Conshohocken PA – InfoMC, a leading provider of clinical and financial management software for care coordination, is very pleased to welcome PACE@Home as a client and partner in bringing state-of-the-art technology solutions to programs serving one of the fastest growing healthcare demographics – individuals who are “dual eligible.”

The Medicare Payment Advisory Commission (MedPAC) defines dual eligibility as qualifying, in some way, for both Medicare and Medicaid coverage. Participants must navigate both systems to access services, and rely on Medicaid to pay Medicare premiums and cover critical benefits Medicare does not cover, such as long-term care. A May 2011 report by the Kaiser Family Foundation estimates that nearly 9 million Medicaid beneficiaries are dual eligible and incur in excess of $300 billion annually in healthcare spending.

Since their inception, PACE (Program of All-inclusive Care for the Elderly) programs have been consistently recognized as a therapeutically-successful and cost-effective approach offered by the Centers for Medicaid and Medicare Services (CMS) for serving a traditionally vulnerable and expensive segment of the population. PACE models of care help eligible patients achieve greater independence at a much lower cost by extending the availability of home and/or community-based care and reducing the need for hospitalization or transfer to a residential nursing facility.

Located in Newton, Catawba County, in western North Carolina, PACE@Home is a program of all-inclusive care for the elderly that allows individuals in need of skilled nursing home care to remain in their home or community setting. PACE@Home is a voluntary, medical/social-based model of care for those who are eligible.

PACE@Home creates an individualized plan of care for each participant and delivers all needed medical and supportive services. This person centered plan of care is developed, reviewed and updated regularly by an Interdisciplinary Team to insure the participant’s needs are addressed. Services may include:

  • Adult day health services
  • Medical care and medical specialists
  • Dental, vision, hearing and foot care
  • Meal planning and nutrition services
  • Home healthcare and personal care assistance
  • Medications
  • Social Services
  • Respite care
  • Physical, occupational, and speech therapy
  • Recreational and social activities
  • Care-related transportation
  • Hospital care, emergency services and nursing facility care

PACE@Home CEO Jane Rollins, who is also Chair of the North Carolina Adult Day Services Association, says, “We chose InfoMC technology to help manage our business for many reasons – not the least of which is the ability to efficiently and accurately address all our clinical documentation, care coordination, contract management, regulatory reporting, and financial transaction needs. Most important to me, however, was the obvious commitment of their staff and leadership to a true partnership with us, not just a customer-vendor relationship.”

JJ Farook, InfoMC CEO, believes his company’s software solutions can help define the next generation of PACE and other dual eligible programs. “Our technology combines managed care functionality and clinical care data to efficiently meet the business needs of a PACE program as a Medicare Advantage plan – whose enrollees are by definition dual eligible – and provides a way to integrate clinical care information in a usable record for clinical staff on a single platform.  PACE plans use many forms to collect and transmit patient information. Incedo’s unique capability to turn data into useful information for the entire PACE team helps improve clinical care,” he said.

The PACE model is centered around the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. It is based on a flat capitated payment system that allows PACE providers to respond to the needs of each participant, rather than a fee-for-service system.

According to the National PACE Association, the PACE model of care can be traced to the early 1970s and an effort by Dr. William Gee, a public health dentist, and Marie-Louise Ansak, a Swiss-born social worker on staff at San Francisco General Hospital, who developed a program for addressing the long-term-care needs of immigrant families in San Francisco. Called On Lok (Cantonese for "peaceful, happy abode"), by 1990 it had evolved into the first federally-funded PACE programs. Creation of PACE programs steadily increased across the country for the next two decades. Today, there are more than 80 PACE programs operating in 29 states.

Media Contact:  Angela Madara, angelam@infomc.com, 484.567.0603

August 22, 2012 – Conshohocken PA – InfoMC, a leading provider of process improvement technology for healthcare organizations, announced today that it has acquired the intellectual property rights to MyRecoveryPlan, a peer support tool developed by the Depression and Bipolar Support Alliance (DBSA) in Chicago, Illinois.

MyRecoveryPlan is a consumer-facing portal that will provide InfoMC clients the ability to offer online peer support services to their members. The software offers a range of standardized tools and resources to help inform, educate, engage, and empower consumers.  These are provided in an easy-to-use online environment that enables recovery plan creation and monitoring, social networking, and promotes peer coaching and support.

The program is based on the premise that mental health recovery means finding hope that life can change for the better by setting and accomplishing goals, overcoming stigma, learning to minimize the negative effects of illness, and being a full partner in medical or mental health treatment. It was developed collaboratively by DBSA and researchers at the Group Health Research Institute in Seattle and supported by a research grant from the US National Institute of Mental Health (NIMH).

DBSA President Allen Doederlein says, “We are delighted to join forces with InfoMC and get this tool out to a broad user base. InfoMC is an organization with a real commitment to the health and well-being of their clients and the individuals they serve.  With InfoMC as a partner, DBSA looks forward to expanding the reach of critical services and resources to the millions of people seeking mental health and overall wellness.”

A number of services and tools are embedded in MyRecoveryPlan. These include, but are not limited to, interactive educational information (text, slide shows, video testimonials) about the concept of recovery and creation of a recovery plan; tools to create, store, and update a personal recovery plan; tools to support self-monitoring of recovery progress and warning signs; and community discussions using chat rooms and discussion groups. Personal online coaching from a certified peer specialist will also be available.

Kathy McAleer, Vice President of Product Strategy for InfoMC, is excited about the new product and the extended capabilities it will bring to their clients. “We believe that online peer coaching and support has the potential to increase both the effectiveness and efficiency of the peer coaching process. There is evidence to support increased engagement when peer coaching is complemented with a standardized online tool set. MyRecoveryPlan will be deployed through the InfoMC platform and can be customized to fit a full range of coaching models and organizations,” she says.

For organizations that are new to peer support services and coaching, InfoMC can help them develop training and certification for their coaches. InfoMC looks forward to helping managed care plans, commercial health plans, integrated delivery systems, inpatient hospital facilities, employee assistance programs, and consumer-run organizations make this important and valuable recovery tool available to their respective constituencies.

InfoMC Introduces Technology for Coordinated Care and Analytics

June 20th 2012

Incedotm Boasts Features and Functionality to Address Emerging Standards and Requirements

Media Contact:  Angela Madara, angelam@infomc.com, 484.567.0603

June 20, 2012 – Conshohocken PA – InfoMC, a leading provider of process improvement technology for healthcare organizations, is pleased to introduce an array of new products and solutions for health and human services organizations. Widely known for their innovative approach to managed care and employee assistance programs, InfoMC also provides technology solutions for care coordination, population/disease management, and business intelligence and data analytics in behavioral health managed care and other chronic conditions.

“As the U.S. healthcare system progressively moves away from reactive medicine towards a population-based proactive model of wellness and prevention, Accountable Care Organizations (ACO), Coordinated Care Organizations (CCO), and Health Homes are rapidly becoming the norm in patient-centered care. InfoMC solutions help these organizations be nimble and responsive in addressing ever-evolving regulatory standards, definitions of treatment, and outcomes measures,” says InfoMC CEO JJ Farook.

Predicated upon the principles of Engage, Exchange, Transact, and Analyze, the Incedo product suite is a plug and play software solution that can work with legacy systems.  Incedo enables clinical and financial management for chronic illness care coordination and wellness across the entire population. Incedo also supports behavioral and physical health integration through the use of workflow automation, clinical intelligence, and evidence-based best practice guidelines.

Care Coordination

Coordinated, person-centered care involves groups of providers willing to be accountable for the quality, cost, and overall care for a designated population. Unlike the last generation of managed care, these networks collaborate to help improve the overall health of their patients, provide better care, and reduce costs while achieving measurable outcomes. The InfoMC Coordinated Care technology solution helps determine individualized care plans, select the best resources for relevant and unique services, facilitate proper placement in the community, provide decision support tools for care managers, and evaluate their performance.

Chronic Care Management

As healthcare costs continue to rise, every segment of the healthcare system is under increasing pressure to control expenses while managing demands for chronic, high cost populations. The InfoMC Chronic Care Management technology solution allows healthcare organizations to easily identify, engage and measure intervention programs for high-risk populations. The technology solution also gives care managers access to critical health information including a complete view of a member’s clinical and social history across multiple chronic conditions using a robust BI & Data Analytics tool.

Business Intelligence & Data Analytics

The sheer volume of healthcare data and related information has reached a critical mass. New healthcare reform mandates have created added challenges for healthcare organizations at all levels to provide accurate and functional information in a timely manner. The InfoMC Business Intelligence & Data Analytics technology solution creates smarter healthcare organizations by focusing on value, not volume of data.  Users can easily aggregate, analyze, share and act on critical information to improve business and health outcomes and reduce costs.

“We see these components as vital to successful healthcare strategy for patients, clinicians, administrators, and regulators. We’ve made sure our solutions seamlessly integrate and exchange information with third-party products and systems, including electronic health records, state and county systems, and payor organizations. And they deploy with ease in cloud or on-premise installations and mobile or desktop devices,” adds Mr. Farook.
